Dr Lin,

I have chronic pain and sensitivity in my left testicle. The veins behind the testicle are swollen and sensitive. Sometimes it swells up and the blood supply to the testicle is cut off and I am in severe pain for a few hours and must lay down until the blood flows again. The severe pain was often brought on by sexual arousal that was not followed by sex. If I walked with my girlfriend and we kissed and I kept walking I would leak excessive fluid and the testicles became very sensitive and often a severe pain episode would follow. However sometimes they came without any reason, it is painful and tight along the inguinal ligament and it makes walking and exercise painful. Nerve pain runs down the inside of my left leg.

When the pain first began I was diagnosed with epididymitis and took antibiotics for 3 months. At first it helped but the pain has continued for 2 years. The last urologist I saw said there is inflammation in the testicle as the ultrasound was fine.

I have written up an extensive history that I give to therapists as I was having trouble remembering all the problems.

October 2001
Practiced jelque and PC muscle exercises for a short time. After 2 months I noticed erections were not as strong or easy to get so I stopped jelque. Continued to practice PC muscle exercises for some time afterwards. 
Masturbated 2-3 times a week. Was not having sex.

Early 2003
Started experiencing dull ache and heat in testicles after work. Working in IT sitting for approx 8 hours a day. Experimented with pillows and seating position to prevent the ache. Erections had been weak since the jelque, low sex drive. Masturbated once a week, took a long time to ejaculate with weak erection. No sex.

Mid 2003
At some point started fitness program, lost approx 20 kg. At home push ups and sit ups. Started gym work.

July 2003
Saw GP about testicle pain. Ultrasound revealed nothing wrong. No action or follow up recommended.

Early 2004
Experienced severe pain in hip moving down to groin while on cross trainer at gym. Very tender to walk however was fine the next day.

March 2004 
Since the pain began in the testicles small blood vessels appeared over time near the surface of the skin on the scrotum. After showering one of these had burst and was bleeding a small amount.

That day I had the first pain episode at work. The pain happened after a lunch time walk. I had walked with a girl I liked and we had decided to start dating that day. I had become excited and a bit of fluid leaked from the penis.

When I sat down at the computer the pain started on the back left hand side above the hip. At the front it ran from the top of the left hip down the inguinal canal too the left testicle. Across the abdomen the inguinal felt like it was very hard and hot. In spasm. The pain was very severe, it felt like I had to urinate so went to the toilet and also to look at what was happening. Could not pass urine, vein running to left testicle was very wide and very hard as if blood supply to the testicle was stopped. It felt like it could burst. A work colleague drove me to Box Hill hospital. By the time doctors looked at me the episode had ended, 3-4 hours since it first began. Was then able to pass urine. Apart from being very tender when moving was no longer in severe pain. Spermatic chord very tender to touch.

Doctors diagnosed me with epididymitis. One doctor thought varicocele due to redness and heat in the left testicle. Urologist prescribed a 3 month course of antibiotics. Initially seemed to help however pains still occur.

Since then any movement that put strain on middle of body hurt. Approximately 15 – 20 more pain episodes experienced since then. One or two happened on the right testicle. Sometimes the pain was had on the inguinal area that did not involve testicle pain, this was associated with a bowel movement. 

They could be brought on by walking longer distances, sitting for long periods of time, becoming sexually aroused and not following through with sex. This combined with walking in particular caused an episode quite reliably. Sometimes I was not sure what brought it on.

May 2004
Began having sex with girlfriend, first time having sex for me. Weak erections and were difficult to get. Extremely painful in testicles during petting or foreplay. There was no pain on or after ejaculation. Rarely masturbating. Sex every few weeks.

July 2004
First physiotherapy work helped me walk without pain, muscles were very tight and sore. Then began to regularly see osteopath who has helped manage and get lots of movement back. Muscles are much better. Less likely to set off a pain episode doing exercise etc… If one does occur recovery is much faster.

August 2004
Erections were now much stronger and upright. Sex was lasting much longer with much more control, able to have sex 3 times in a row. Usually had sex 3 times once a week. Testicles felt much better after ejaculating.

April 2005
No longer in relationship, no sex. Masturbating every day. If masturbation was left for a few days testicle was more painful.

August 2005
Went on holidays and on tour done a lot of activities without any trouble. Upon returning to work after 2 days I had the last pain episode. Masturbating 2-3 times a week to avoid testicle pain.

October 2005 
Resigned from work due to ongoing problems sitting.
Have not experienced any pain episodes since resigning, however the swelling and pain in testicle is still present.

February 2006
Saw Urologist again. He noted swelling behind the left testicle. Ultrasound came back ok and he suggested inflamation was the problem. Masturbating once a week, stings when ejaculating. Sometimes it stings when urinating. Ache and heat continues in left testicle, inguinal ligament very tight.
